The Cardiovascular
MR and CT research and clinical unit at Forsyth Radiological Associates
(FRA) and at the Forsyth Medical Center is headed by Dr Duerinckx.
He is the Director of Cardiac MR, CT and Nuclear medicine for FRA
and for the Medical Director for Cardiac CT & MRI for the Radiology
Department at Forsyth Medical Center. He has co-authored over 130
publications and four text books.
We offer short-term
(one week to 3 months) and long-term (up to 12 month) Visiting
scholarships for clincial radiologist and cardiologists and
for Physicists to do research, observe and train in advanced cardiac,
vascular and body MRI & CT techniques at Forsyth Medical center
and affiliated Imaging centers. This fellowship program is
not related to any other fellowship programs offered at the Forsyth
Medical Center or Wake Forest University School of Medicine.
For
Clinical Training fellowships we charge from $10,000 to $15,000
per month, and can possibly help you with COCATS II level training
in Cardiac CT & MR for US physicians with a valid medical license
in North Carolina. This is a FLEXIBLE program, where you structure
multiple visits of any duration to amount to about 20 days (= 1
month equivalent), as needed based on your clinical practice demands.
SIEMENS
Sponsored one week (or a random number of days) long Clinical Fellowships
are like extended "Site Visits" and allow clinicians and
or technologists interested in the near or future purchase of a
Cardiac CT or MR scanner to work with Dr Duerinckx for up 2 hours
per day during up to 3 afternoons per week, with additional acitivities
organized the rest of the week, and extensive visits and discussions
with local users of this equipment. Please contact your local Siemens
rep to arrange for funding for these event (Cost: $1,000/day for
physicians; 50% discount for technologists if accompanied by a physician).
For
all other fellowships (with purely 100% research focus): No salary
support is provided. The visiting scholars
need to have their own financial support. It is expected that all
scholars actively participate in on-going research programs and
co-author one or several research publications during their visit.

LONG TERM
VISITING RESEARCH FELLOWSHIPS:
During such RESEARCH fellowships there is the opportunity to participate
in clinical work, but only for US Citizens with a valid USA Medical
License. A North Carolina medical license is required for any work
at Forsyth Medical Center. Also, full hospital credentialing will
be needed before you can participate in clinical work.
Fellows without a NC Medical license, or without hospital credentials,
can still observe clinical work, but not actively participate.
Clinical and
research Experiences
Most
of the time will be devoted to RESEARCH, including participation
in the write-up of research studies, papers and manuscripts, review
articles, etc...
Hands-on
Cardiac and Vascular clinical MRI and CT and MRA and CTA experience
will be gained as follows. One to two day per week is devoted to
clinical MRI and MRA studies (usually Tuesday and thursday). Three
days per week will be devoeted to Coronary CT Angiography (Mon-Wed-Fri).
Unlimited time and evenings will also be reserved for research studies.
Additional pateint centered research can be performed during evenings
and early mornings (before 7 AM and after 9 PM) as needed.
Scholars can
attend many clinical and research conferences at Forsyth
Medical Center, as well as at other medical centers in Winston
Salem (Wake Forest University
School of Medicine, formally Bowman Grey School of Medicine and
its hospital: the Wake Forest University BAPTIST Medical Center).
We also have
a 3-D Imaging laboratory and two working Picture Archiving and Communication
System (PACS) in the Radiology service. Scholars can learn about
our PACS.
FRA or Forsyth
Medical Center Certificate of "Visiting Scholarship".
FRA or Forsyth
Medical Center can provide a certificate of "Visiting Scholar"
upon request if certain conditions are met (and only for people
with a Medical Doctor Degree). These positions are very different
from "Traditional Medical Center Radiology fellowships",
which are highly competitive training positions and are given for
12 months only. The candidate needs to submit a full curriculum
vitae and reprints of recently published articles (up to 5) to Dr
Duerinckx. The request will then be submitted to and discussed by
a Radiology Department committee at Forsyth Medical Center Radiology
at their monthly meetings. If approved by this Radiology committee,
then FRA or the Forsyth Medical Center could issue a certificate
after successful completion of the scholarship training. This initial
approval process takes up to 2 months to complete.
No certificates
are available for Medical students or undergaduates.
The practice
of medicine in North Carolina.
The North Carolina
Medical Board issues medical licenses in North Carolina. Long
term fellows are encouraged to apply for a license and possibly
even hospital credentials, as needed. The North Carolina General
Statutes require that you must have a valid North Carolina medical
license to practice medicine in North Carolina. We recommend that
you do not make any commitments based on expectation of licensure
until you are actually licensed. The North Carolina Medical Board
annually processes over two thousand applications for medical licensure.
The process involves the collection of credentials from you the
applicant, and from other sources as well. Once all application
materials have been collected, the NCMB staff must review them.
After reviews have been done, it may be necessary for the staff
to request additional information, and/or to return items that need
corrections or clarifications. It is the goal of the NCMB to review
materials as quickly as possible, but they must be thorough. You
should not expect the entire process to take less than9-12 weeks
from the time your application is received by the NCMB. In 2004,
a fee of $288.00 U.S. dollars is to be paid at the time the application
is submitted. ($250.00 for the license application fee and $38.00
for the criminal background check fee). If you also request a temporary
license, an additional fee of $150.00 is required (please check
NCMB web site,
as rates may change).

For
FOREIGN Visitors only:
Any incidental
patient contact involving the alien or out-of-state physician (i.e.,
visiting scholar from outside USA or out-of-state) will be under
the direct supervision of a physician who is licensed to practice
medicine in the the state of North Carolina and in the USA.
Inexpensive
AIR TRAVEL can be arranged via discount airlines or American Airlines
Immigration
Requirements - It is up to the scholar to get a valide US Immigration
Visa status. It used to take 3 months for J-1 Visas; given new
immigration security concerns, this may now take longer.
PROCEDURES:
The applicant is responsible for obtaining his/her own visa. We
can not provids guidance about which visa ÒmightÓ be appropriate.
Individuals requesting a training rotation of one year or more must
apply to the U.S. Department of State. We are NOT an authorized
sponsor for an IAP-66.

You must obtain a visa before attending long-term courses at our
institution. You have two options depending on the length of time
you intend staying.
Option A:
a) If you are participating in an educational course or an Observer
Training program for (6) months or less, a B1/B2 visa is sufficient.
This visa is intended for business/tourist visitors to the U.S.
There are generally no restrictions associated with this visa except
that it expires within 6 months.
Option B:
b) If you are participating in an educational course or an Observer
Training program that lasts longer than 6 months, you will need
a J1 visa. This visa comes with numerous restrictions that you must
know and understand.

Hospital Identification
BADGES while visiting
At Forsyth Medical
Center you will receive a VISITOR badge (with photo) and at the
FRA you will get a Volunteer badge (also with photo). The radiology
department at Forsyth Medical Center will provide the needed paperwork
to get your badge. At FRA you will get a VISITOR- Visiting Scholar
(or similar temporary) badge.
HOUSING
during your stay in Winston-Salem, NC.
You have several
housing options. You can rent directly in the local community (get
a copy of the Winston-Salem
Journal our local newspapers, which in their Sunday edition
lists many apartment and house rentals), or ask the Forsyth Medical
Center Housing office to help you find a rental unit. Unfortunately,
rental vacancies are often short-term, and one can not guarantee
availability several months in advance. This often presents a practical
problem.
Option #1: Apartment
RENTALS in the local community
Forsyth Medical
Center does NOT provide special housing assistance for foreign visiting
faculty members and scholars.
Housing costs
vary and go from $800/month for a very small one-bedroom apartment
to $1,500 for a nice 2-bedroom apartment close to Forsyth Medical
Center (rates need to be checked.... and may be inaccurate as of
December 2004). Houses can be rented from $1,100 to much higher.
IF
YOU DECIDE TO RENT A ROOM IN A HOUSE, BE EXTREMELY CAREFUL AND ASK
MANY QUESTIONS.
Renting a room is an inexpensive option (US $ 500 and up/month),
but could turn out to be very unpleasant. Please check : Will you
have access to a phone? Will friends be able to call you (some landlords
refuse to take calls for renters) ? Will the landlord take messages
for you? How much will it cost to install
a second phone line (if needed)? Will you have to share a bathroom?
